Intake survey
The registrar's computer software appears to be incapable of checking the
prerequisites for this class, so we will have to use permission codes
for several students. In order to get a permission code, you must
fill out this intake survey. (We want an intake survey from everyone,
but the permission code is a stick we can use to guarantee getting some.)
