Intake survey
The registrar's computer software is incapable of checking the
prerequisites for this class, so we will have to use permission codes
for several students. In order to get a permission code, you must
fill out this intake survey. (I want an intake survey from everyone,
but the permission code is a stick I can use to guarantee getting some.)
